Fibroids focus: A guide to imaging

Presentation Synopsis / Abstract

Known by multiple names, fibroids are the most common gynaecological tumour in persons assigned female at birth. Although some patients are asymptomatic, heavy menstrual bleeding, infertility, and pelvic pain with and without the feeling of heaviness can greatly impact the lives of symptomatic patients. Ultrasound, specifically transvaginal ultrasound, has long been the preferred imaging modality due to the cost effectiveness, availability and spatial resolution.

The Federation Internationale de Gynecologie et d'Obstetrique (FIGO) classified fibroids using standardised terminology for systematic reporting more than 10 years ago.

Using the pneumonic CAPS-F as a guide, sonographers can systematically count (C), describe the anatomical (A) location, proximity (P) of the fibroid to the endometrium or cornu, assess the size (S) and assign a FIGO (F) classification to systematically record and report fibroids.

In this lecture the systematic reporting of fibroids using FIGO will be discussed and outlined, including tips and techniques which can be applied during routine pelvic ultrasound.

Biography

Dr Jennifer Alphonse FASA | Central Queensland University Dr. Jennifer Alphonse is a generally trained sonographer with a passion for education, research and advanced clinical practice. Jennifer has worked in general practice, a dedicated paediatric practice and for more than 20 years in a specialist O&G practice. In 2015 Jennifer completed a PhD which included five publications. From 2013 to 2019 Jennifer was a research sonographer, extending to an adjunct lecturer appointment at University of NSW until 2022. Currently Jennifer is a senior lecturer at CQU and Jennifer and is currently co-supervising both Masters and PhD Candidates enrolled at CQ University. Jennifer is a manuscript peer reviewer, well published and currently working on research publications. Jennifer enjoys teaching and sharing knowledge and has a particular interest in gynaecological ultrasound having been heard to say “I love an empty uterus” in reference to her enthusiasm for high level gynaecological/pelvic ultrasound. In 2024, Jennifer was the recipient of the ASA Pru Pratten Memorial Lifetime Achievement Award.

Beyond the niche - understanding isthmoceles **NEW VOICE**

Presentation Synopsis / Abstract

Isthmocele, a defect at the site of a previous caesarean section incision, is frequently misinterpreted sonographically as simple fluid. However, isthmoceles represent a significant pathological entity with the potential to cause considerable morbidity if left unrecognised. Their consequences extend beyond abnormal uterine bleeding and pelvic pain, posing serious risks to fertility and future pregnancies.

This presentation aims to enhance sonographers’ understanding of isthmocele anatomy and definition, outlining the underlying structural changes at the caesarean scar site. It will review the common symptoms, complications, and recognised risk factors associated with this condition.

Emphasis will be placed on optimal diagnostic techniques, particularly the role of ultrasound as the first‑line modality. Additional imaging strategies will also be briefly discussed.

The session will provide guidance on accurate measurement and classification of isthmoceles using sonographic criteria. Finally, a recent clinical case will be presented to illustrate typical imaging findings and highlight the importance of timely diagnosis.

Biography

Mrs Brooke Fagerstrom | Coffs Harbour Hospital Brooke is a dual qualified as both a radiographer and sonographer. She graduated from Medical Radiation Science in 2014 and completed her Post Graduate Diploma in Medical Ultrasound in 2019. Brooke commenced work and completed her ultrasound training in Private Practice. She now works in the public health system at Coffs Harbour Base Hospital. Brook enjoys the hospital environment and engaging in a mix of radiography and ultrasound shifts. She recently moved to the Mid North Coast to enjoy the coastal lifestyle. She is passionate about obstetrics and gynaecology and would like to complete her Masters in ultrasound in the future.
