Untwisting the diagnosis: Ultrasound in adnexal torsion

Ovarian torsion affects 2-3% of women seeking emergency department because of pelvic pain. Key sonographic features are enlarged, edematous ovary, peripherally displaced follicles with a follicular-ring sign, “whirlpool sign (visualization of a twisted vascular pedicle) and possible free fluid in pouch of Douglas. Absence or reduction of intra-ovarian blood flow may support the diagnosis, however, normal Doppler blood flow cannot exclude torsion due to collateral blood supply and absent Doppler blood flow cannot confirm the torsion.
The lecture will cover risk factors, pathogenesis and diagnostics of ovarian torsion and include plenty of ultrasound images illustrating ovarian or adnexal torsion.

Ligita Jokubkiene is associate professor at Lund university, Sweden and senior consultant in obstetrics and gynecology at the Department of Obstetrics and gynecology, at Skane University Hospital in Malmö, Sweden.
Ligita Jokubkiene is an expert in gynecological and obstetrical ultrasound. She has defended her thesis with the title ”Three-dimensional ultrasound studies of normal and abnormal ovaries” in 2012. Currently she is an active researcher leading several research projects with focus on ultrasound diagnostics in gynecology, particularly on endometriosis diagnostics and education, pelvic floor birth-related injuries and pelvic pain. She is also participating in international multicenter studies lead by IOTA, IETA and MUSA groups studying ovarian masses and endometrial and myometrial pathology. Ligita Jokubkiene is supervising master and PhD students.
Ligita Jokubkiene organizes and leads national and international courses on ultrasound diagnostics from basic to advanced level. She has been an invited speaker at many international courses and congresses.
Ligita Jokubkiene is a chair of Educational courses subcommittee at ISUOG (International Society of ultrasound in obstetrics and gynecology), Advisory board member at European Endometriosis league and Board member at IOTAplus. She is also a chair of the Ultrasound reference group at Swedish Society of Obstetrics and gynecology in Sweden.
Ligita Jokubkiene is a chief supervisor of the medical Students at the Faculty of Medicine, Lund University.

Subtle signs of ovarian cancer: From solid areas to papillary projections

We all get that sinking feeling when we first see an ovarian mass on transvaginal ultrasound and in the back of our minds we are thinking "what is that?". Our training kicks in and we start with the basics:
1. Where is it?
2. How big is it?
3. Does it have more than one loculation?
4. What is the echogenicity of the internal contents?
5. Is there wall thickening?
6. Is there vascularity and where is the vascularity?
7. then you come to the solid.....is that a focal solid area or a papillary projection....wait; what is the difference between a "solid area" and a "papillary projection"?
In this lecture images of solid ares and papillary projections will be shown. By the end of this lecture you will hopefully have greater confidence reporting a solid area versus a papillary projection in an ovarian mass.

Dr Jennifer Alphonse FASA |

Dr. Jennifer Alphonse is a generally trained sonographer with a passion for education, research and advanced clinical practice. Jennifer has worked in general practice, a dedicated paediatric practice and for more than 20 years in a specialist O&G practice. In 2015 Jennifer completed a PhD which included five publications. From 2013 to 2019 Jennifer was a research sonographer, extending to an adjunct lecturer appointment at University of NSW until 2022.
Currently Jennifer is a senior lecturer at CQU and Jennifer and is currently co-supervising both Masters and PhD Candidates enrolled at CQ University. Jennifer is a manuscript peer reviewer, well published and currently working on research publications.
Jennifer enjoys teaching and sharing knowledge and has a particular interest in gynaecological ultrasound having been heard to say “I love an empty uterus” in reference to her enthusiasm for high level gynaecological/pelvic ultrasound. In 2024, Jennifer was the recipient of the ASA Pru Pratten Memorial Lifetime Achievement Award.

Ovarian hyperstimulation syndrome

Ovarian Hyperstimulation Syndrome (OHSS) is an uncommon iatrogenic condition associated with ovarian stimulation, particularly in the presence of human chorionic gonadotropin (hCG). It is generally a self-limiting condition, and the majority of patients can be managed as outpatients.

Typical and atypical endometriomas: Diagnostic clues and challenges

Ulrasound is a first-line method to diagnose endometriomas. Typical endometrioma looks line a unilocular cyst with ground glass echogenicity. Approximately 50% of endometriomas exhibit this appearance at ultrasound. Some endometriomas present with less common features and can be multilocular, have low-level echogenicity or be anechoic, and may include calcificates. Endometriomas may change during pregnancy and develop papillary projections that are highly vascularized and may mimick borderline ovarian malignancy. This process is called decidualisation.
The lecture is covering typical and less typical appearance of endometriomas at ultrasound as well as changes of endometriomas during pregnancy.
