Uterine artery Doppler is a key component of contemporary screening for pre-eclampsia and plays a central role within the Fetal Medicine Foundation (FMF) first-trimester algorithm. This presentation will provide a practical, evidence-based update on the technique, interpretation, and clinical integration of uterine artery Doppler in routine care.
We will review optimal acquisition techniques, including transabdominal and transvaginal approaches, correct identification of the uterine arteries, and common pitfalls that impact reproducibility. Emphasis will be placed on the measurement and interpretation of the pulsatility index (PI), using gestational age-specific reference ranges and quality assurance principles.
The role of uterine artery Doppler within the FMF combined screening model—integrating maternal factors, mean arterial pressure, uterine artery PI, and biochemical markers—will be discussed, highlighting its contribution to risk stratification and targeted prevention of preterm pre-eclampsia with aspirin.
We will also address the interpretation of abnormal findings, including elevated PI and notching, and their clinical relevance within both first- and second-trimester assessments. Finally, the session will focus on translating Doppler findings into clinical decision-making, including counselling and identification of women who would benefit from preventive strategies.
This presentation aims to bridge technical excellence with evidence-based application, ensuring consistent and effective use of uterine artery Doppler in clinical practice.

Ms Andrea Arnold |
Maternal Fetal Medicine GCUH
Andrea Arnold is a Sonographer Consultant with 26 years’ experience, including 18 years dedicated to women’s health. She has worked in Maternal Fetal Medicine for the past 11 years, joining the MFM team at GCUH in 2015. Her career also includes a four-year clinical hiatus as an applications specialist, broadening her technical, educational & business expertise. Andrea has extensive experience in obstetric imaging, advanced fetal cardiology and pre-eclampsia screening, with a particular clinical interest in fetal growth restriction and fetal echocardiography. Alongside her clinical work, she leads a team of advanced sonographers in a healthcare leadership role and is currently undertaking an MBA as a side hustle.
