POCUS is a rapidly expanding field in the world of ultrasound.
With all specialities now involved in use of point-of-care ultrasound with differing clinical questions and emphasis, how does does a Sonographer bridge the gap between radiology-based ultrasound techniques and teaching methods to specialist and critical medical personnel?
This presentation will highlight some key features of POCUS and give a practical approach to training doctors in the use of ultrasound, and give examples of how understanding POCUS use can elevate the clinical understanding of sonographers in their everyday practice.

Jennifer is the Director of Education AIU Singapore and is Specialist Sonographer and Ultrasound Educator and at Qscan on the Gold Coast, Australia.
Specialities include training all aspects of POCUS, especially Nephrology, Cardiac, Lung ultrasound, and in Specialist MSK and Vascular subjects. She has taught all of these subjects in Singapore, and in The Philippines, Hong Kong, and Japan.
Jennifer has presented in Australia, New Zealand for ASUM, ASA, RACGP,ACRRM, and ACSEP, as well as WFUMB, ASUM Outreach, and Radiology Across Borders for programmes in Cambodia and Fiji.
She was Convenor of the Scientific Committee for ASUM 24 Scientific Conference held in Brisbane in 2024.
Jennifer is also a Specialist MSK Tutor for Professor Carlo Martinoli at Martinoli MSK Ultrasound Courses in Australia, Singapore, The Philippines, and Taiwan.
Jennifer has a Masters of Medical Ultrasound with Distinction (MMedUS), DMU General (ASUM). Certification in Global Health Proficiency -Radiology (RAD-AID), Certificate of Healthcare Leadership (NUS), and Certificate IV in Training and Assessment (Australia).

Update on PoCUS for generalist sonographers and radiology

The rapid expansion of point-of-care ultrasound (POCUS) across emergency medicine, anaesthesia, critical care, and general practice has challenged traditional radiology-centred ultrasound models, raising concerns around scope, governance, and image quality. Yet POCUS has matured into a purposeful stream of practice, driven by focused clinical questions, time-critical decisions, and patient-centred care, offering opportunities for sonographers to develop new skills and explore emerging applications.
This presentation does not debate the competency of non-sonographer users, but examines POCUS across specialties including emergency medicine, neuraxial practice, rehabilitation and pain medicine, and gastric ultrasound. By exploring these evolving inquiries, sonographers can refine techniques, adopt new approaches, and enhance patient care and outcomes.
Viewed through an educational and patient-advocacy lens, POCUS becomes less a threat to traditional models and more a catalyst for innovation. This presentation seeks to reduce professional friction, spark curiosity, and encourage collaborative growth. By understanding and engaging with these shifts, radiology-based practitioners are positioned not merely to adapt, but to help lead the next phase of ultrasound innovation.

Defining competency in point-of-care ultrasound

Background
The rapid expansion of Point-of-Care Ultrasound (POCUS) across diverse clinical specialties has outpaced traditional credentialing models. Historically, competency has been tethered to volume-based metrics (logbooks) and attendance, which fail to capture the multidimensional nature of ultrasound—incorporating image acquisition, interpretation, and clinical integration.
Methods
This presentation explores contemporary approaches to competency, contrasting POCUS’s "rule-in/rule-out" goal-directed nature with the comprehensive diagnostic scope of formal radiology and cardiology. We examine the limitations of current Australian frameworks, such as the CCPU and DDU, and the inherent variability in inter-operator performance and clinical context.
Results
To bridge the gap between "activity" and "true performance," we introduce the ULTRA-Metrics framework. Developed via an international multi-disciplinary Delphi study involving 19 experts (including emergency, intensive care, and sonography), ULTRA-Metrics shifts the focus from scan numbers to observable behaviours. It utilises an "Exam Agnostic" approach and a 5-point Modified Ottawa Entrustability Score, ranging from "Dependent" to "Independent." This framework assesses six core domains: acquisition, optimisation, interpretation, clinical decision-making, communication, and professional scope awareness.
Conclusion
Competency is a dynamic continuum rather than a static checkbox. Transitioning from subjective "vibes" to objective, consensus-driven data through frameworks like ULTRA-Metrics ensures that credentialing reflects clinical safety and diagnostic efficacy. By prioritising quality-based metrics and entrustment, clinicians and sonographers can establish a shared language for education, ultimately enhancing patient care through high-quality bedside imaging

Suean Pascoe, Director of Zedu Ultrasound Training Solutions, is an internationally recognised ultrasound educator dedicated to advancing practical skills development. She leads the creation and delivery of cutting-edge training programs designed to accelerate the acquisition of hands-on ultrasound skills. She also developed the TRUST (Train the Ultrasound Trainer) course, which equips educators with strategies to enhance teaching and mentoring.
